This module is created to assist practices using Reach Out and Read as a strategy to meet multiple KM Competencies for PCMH. Each KM competency, guidance and evidence will be shared to help you provide evidence-based care that supports population needs and provision of culturally and linguistically appropriate services.\n

Learning Objectives:\n

Understand which Knowing and Managing Your Patients (KM) Competencies for Patient Centered Medical Home (PCMH) are supported by Reach Out and Read (ROR).
Learn how to use Reach Out and Read as a strategy and the evidence needed to meet each supported competency.

Certification

This Enduring Material activity, Reach Out and Read Beginning at Birth, has been reviewed and is acceptable for up to 0.25 Prescribed credits by the American Academy of Family Physicians. Term of approval begins 04/30/2025. Term of approval is for one year from this date. Physicians should claim only the credit commensurate with the extent of their participation in the activity.\n

AMA/AAFP Equivalency:

AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1 credit(s)™ toward the AMA Physician's Recognition Award. When applying for the AMA PRA, Prescribed credit earned must be reported as Prescribed, not as Category 1.\n
